From the Home tab of Samsung Health, touch the More Options menu in the upper right corner.
Touch Settings, and then scroll to and touch Erase personal data.
Touch Erase, and then touch Erase again to confirm.
When prompted, enter your Samsung Account information, and then touch OK to finish.
